In consideration of using enrolling your child in the High Five Sports Academy LLC, and for other good
and valuable consideration, the undersigned acknowledges, understands and agrees to the following:
1. I am the legal parent or guardian of the child listed below and give permission for the participate in
the High Five Sports Academy. I certify that he/she is in good health and can take part in all
activities.
2. I acknowledge and am aware that participation in the High Five Sports Academy has some inherent
risks and injury can occur. Risks include, but are not limited to, broken bones, bruises and other
bodily injuries caused by falls or contact with walls, floors or other participants; medical conditions
resulting from physical activity; and damaged clothing or other property. I voluntarily agree to
assume the full risk of any and all injuries, damages or loss, regardless of severity, that I or my
player may sustain as a result of participating in this academy.
3. In the event of an emergency or an injury occurs, I authorize the staff members to take all proper
action and use the emergency service available to the nearest hospital if necessary. I understand that
my personal insurance will be used in this case.
4. I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless High Five
Sports Academy LLC, its owners, employees, and/or agents from any and all claims, demands, or
causes of action which are in any way connected with participation in this academy. I further agree
to indemnify, hold harmless and defend High Five Sports Academy LLC, its owners employees or
agents from any and all third party claims and actions resulting from injuries, damages and losses
arising out of, connected with, or in any way associated with this academy. Should High Five Sports
Academy LLC, its owners, employees or agents be required to incur attorney’s fees and costs to
enforce this agreement, I agree to indemnify and hold them harmless for all such fees and costs.
5. I represent that I have insurance to cover any injury or damage my player may suffer or cause while
participating at the academy and I agree to assume full and complete responsibility for any and all
medical bills arising from my player’s participation.
6. I expressly agree that this agreement is intended to be as broad and inclusive as permitted by law. I
agree that if any portion of this agreement is found to be void or unenforceable, the remaining
portions shall remain in full force and effect.
I HAVE HAD SUFFICIENT TIME TO READ THIS ENTIRE DOCUMENT AND SUFFICIENT TIME
TO CONSULT WITH LEGAL COUNSEL PRIOR TO SIGNING. I FULLY UNDERSTAND THE
TERMS AND FULLY UNDERSTAND THAT BY SIGNING THIS AGREEMENT I HAVE
GIVEN UP CONSIDERABLE FUTURE RIGHTS FOR MYSELF AND THOSE FOR WHOM I
HAVE ASSUMED RESPONSIBILITY. BY SIGNING BELOW, I FREELY AND VOLUNTARILY
AGREE TO THE TERMS OF THIS AGREEMENT.
